1. If you have been laid off from:

• Advanced Technology Services
• Caterpillar East Peoria
• Gray Interplant
• Keystone Steel & Wire
• Volt
• Washington Group International (a.k.a. Division of URS), or
  
• XPac (a.k.a. Export Packaging)

follow these instructions.

2. If you have been laid off from:

• Caterpillar Mapleton, or
  
• Caterpillar Mossville

follow these instructions.

3. If you have been laid off from Caterpillar Morton, Caterpillar Peoria or any other company, follow these instructions.
